Under general supervision, plans and provides a variety of outreach and support services including:
raising awareness through cultural diversity presentations, community education and ensure access to community health care resources and services; works closely with neighborhood leaders to develop intervention and prevention strategies to help strengthen the community; provide necessary support and guidance to clients to ensure they attain and maintain opportunities; and performs other duties as required.
Position is contingent upon grant funding continuing to be renewed/approved.

Recruit community organizations, resource and influencers for support related to program activities.
Identifies specific populations based on health/social service needs and grant deliverables, such as, but not limited to, members of youth, minority populations, and low-income populations.
Develops presentations with health information to school groups, community groups, or specific individuals, families and/or populations based on health/social services needs in a manner consistent with cultural norms and in alignment with grant funding.
Plans and conducts events such as workshops, outreach efforts, and educational presentations for varied audiences.
Presents health information effectively before large and small groups of all ages.
Distributes flyers, brochures, or other informational or educational documents to inform members of specific individuals, families and/or populations based on health/social services needs community.
Provides feedback to healthcare providers regarding improving service accessibility or acceptability.
Supports and works cooperatively with the multi-disciplinary teams.
Prepares survey(s) targeting areas, locations, business relations or priority groups to increase data in relations to grant activities.
Assists in assessing community needs by obtaining and analyzing community data specific to the program.
Identifies healthcare trends in the community, continuously work to expand knowledge on available community resources and make appropriate recommendations to clinical staff.
Maintains confidentiality of files and information in accordance with regulations.
May also perform a variety of clerical duties including, but not limited to, data entry, record management, answering phones, compiling data and submitting statistical reports, drafts and formats various materials for distribution to clients.
Conducts ongoing assessments, engages follow up and personal goal setting with clients to promote positive health outcomes.
Provides referrals to identified programs, community services and various resources; acts as a liaison between clients and community agencies to advocate for the client and ensure the proper services are being received.
May attend client appointments and conduct visits; establishes and maintains communicative relationships with clients and their families.
Identifies barriers to healthcare; collaborates with internal and external stakeholders to ensure the accessibility of services for clients.
Monitors client needs and progress, documents client case management information in databases and maintain a variant of records and files.
Demonstrates courteous and cooperative behavior when interacting with public and staff; acts in a manner that promotes a harmonious and effective workplace environment.

EDUCATION AND
Experience:
High school diploma or GED OR an equivalent combination of education, training and experience as determined by Human Resources.
REQUIRED CERTIFICATES, LICENSES, AND REGISTRATIONS:
Nevada Driver's License.
Community Health Worker Certificate or obtained within 12 months of hire date.
REQUIRED KNOWLEDGE AND SKILLS Knowledge of:
Standard office practices and procedures, including filing and record keeping principles and practices.
Business arithmetic.
Correct business English, including spelling, grammar, and punctuation.
Community agencies and other outside resources available to clients.
Computer applications related to the work.
Techniques for dealing with a variety of individuals from various socioeconomic, ethnic, and cultural backgrounds, in person and over the telephone, often where relations may be confrontational or strained.
Communicating effectively in oral and written forms.

SUPERVISION RECEIVED AND EXERCISED:
Under General Supervision - Incumbents at this level are given assignments and objectives that are governed by specifically outlined work methods and a sequence of steps, which are explained in general terms.
The responsibility for achieving the work objectives, however, rests with a superior.
Immediate supervision is not consistent, but checks are integrated into work processes and/or reviews are frequent enough to ensure compliance with instructions.

Supplemental Information CONDITIONS OF EMPLOYMENT:
This is a grant funded, part time position.
Position is contingent upon grant funding continuing to be renewed and approved.
Carson City part time employees cannot exceed 1039 hours in a fiscal year.
Continued employment is contingent upon all required licenses and certificates being maintained in active status without suspension or revocation.
Any City employee may be required to stay at or return to work during emergencies to perform duties specific to this classification or to perform other duties as requested in an assigned response position.
This may require working a non-traditional work schedule or working outside normal assigned duties during the incident and/or emergency.
Employees may be required to complete Incident Command System training as a condition of continuing employment.
New employees are required to submit to a fingerprint-based background investigation which cost the new employee $56.

Part-time employees do not contribute to social security; however, they must contribute 7.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
